Practical Information for Your Visit
Fees
Sessions are $150, for approximately 50-55 minutes, when you pay at the time of service. Please allow about 75 minutes for your initial session.
Many of my patients are able to regain their health in a few visits and avoid endlessly taking expensive medications and supplements or undergoing surgery.
Forms of Payment
I accept cash, checks, Visa and MasterCard, and most debit cards.
Gift Certificates
Gift certificates are available for all occasions. Please ask.
Medical Insurance
I am a Blue Cross Preferred Provider. If your health/medical insurance covers massage, it should cover my services, provided you have a written referral from a physician or chiropractor. This written script should have the doctor’s NPI number, and specify the number of treatments and a period of time. If you have any doubts about your coverage, it’s always best to check with your insurer first, before scheduling.
I'll also need the name of the insured, policy number, your date of birth or social security number, the name and contact information of the insurance company and claims adjustor, the claim number, and the date of loss.
Motor Vehicle Accidents
If you have been in a motor vehicle accident, and your vehicle was insured, your treatment should be covered, provided that you first obtain a written prescription from a physician or chiropractor. The script must include the doctors NPI number, the diagnosis and the appropriate ICD-10 codes and should specify the number, frequency, and duration of treatments.
I'll also need the name of the insured, policy number, your Date of birth or social security number, the name and contact information of the insurance company and claims adjustor, the claim number, and the date of loss.
